Trinity Health’s southeast Michigan hospitals partner for community health survey

Every three years, non-profit healthcare facilities are required by the federal government to conduct Community Health Needs Assessments (CHNA), including surveying their communities.

For the first time, the five Trinity Health hospitals in Southeast Michigan, including Trinity Health Ann Arbor, Chelsea Hospital, Trinity Health Livingston, Trinity Health Livonia and Trinity Health Oakland, are partnering to conduct a 2026 CHNA survey. The results of the survey will be used to assess, prioritize and address the needs of the respective communities.

By working together as a system, Trinity Health’s Southeast Michigan hospitals can more effectively leverage its regional resources while also working to address the areas identified within the surveys.

By working closely with community partners, Trinity Health Michigan continuously evaluates the changing health and social needs of our communities. Surveying our communities every three years and collecting input ensures all voices in our respective regions are heard, especially those experiencing poverty and other vulnerabilities.

Once completed, the CHNA reports will prioritize focus areas for each hospital and will be shared publicly on our website.

“The needs identified by the community through the CHNA process guides our local strategic initiatives every three years,” said Dr. Chiquita Berg, vice president of community health and well-being for Trinity Health Michigan. “We are excited our Southeast Michigan hospitals are combining efforts to conduct a single survey and working collaboratively so each hospital gains a deeper understanding of the communities we serve.”

Anyone who lives in the communities of Trinity Health’s Southeast Michigan hospitals — Livingston, Oakland, Washtenaw and Wayne counties — are encouraged to complete this anonymous survey. The survey will remain open through July 31, 2026. There is an opportunity at the end of the survey to submit your name and contact information for a chance to win one of two $100 gift cards that were generously donated by a community partner. Any names and contact information provided will remain confidential and will not be linked with survey answers.
